Community Brain Injury offers a case management program to assist the individual and the family of the individual to identify and develop a plan and find support. The case management program is client-centered and client-driven. A client can create his or her own goals and is empowered to make decisions. The Case Manager meets with each client in the environment that best suits their individual needs, whether in the home, office or a community location such as a library or restaurant. Case Management Services provides services to individuals over the age of 18 with a documented history of an acquired brain injury on the Hampton Roads Peninsula.
CBIS Case Managers work with our clients to connect them with an array of services including
- Safe and secure housing
- Financial Benefits
- Medical, mental health, and rehabilitative services
- Legal Services
- Family Support Services
- Any other support services that the client and/or their family system needs and requests.
