Patients must be able to sit upright in a standard coach airline seat, have a verified appointment, have a physician's medical release to travel, and have a verified financial need. Patients cannot have a communicable disease.

Intake Process
Call to request transportation or apply online at least 21 days prior to the date of the travel. A completed medical clearance form from the doctor who had recently seen the patient is required.
