Adult Day Healthcare Center - Chantilly

The Agape Adult Day Healthcare Center will serve the needs of adults aged eighteen and older who require and may benefit from the programs offered and care provided. This includes frail elderly or disabled adults who do not need to be institutionalized, but need supervision during the daytime while family or caregivers are at work or in need of respite care; those living alone to prevent isolation or need assistance with ADL, are home-bound or those experiencing impairments or changes associated with aging. Capacity is 60.  Services include:

• Daily transportation to and from the center
• Activities ranging from crafts to exercise, from cookouts to trips to local sites, bingo game to sing-a-long and karaoke
• Structured dementia programming
• Specialized program for adults with disabilities or limitation of function
• RN supervision
• Available health services: Doctor comes weekly on site, support to visit doctor's office    
  
  Individualized multidisciplinary plan of care
• Extensive volunteer program
• Individualized breakfast, snack and lunch served daily
• Meals can be modified for special dietary restrictions
• Rehabillitation therapy
• Therapists available when ordered by physician
• Support services for participants and family members
